NHS Long Term Plan
The NHS Long Term Plan sets out the key changes you can expect to the healthcare system over coming few months and years. Local NHS organisations are working with their partners to turn the ambitions in the plan into improvements in services in every part of England.
The plan includes a range of targets which can be achieved in part, through individual level behaviour change, for example: reducing heart attacks and strokes, providing exercise programmes to tens of thousands of patients, and helping patients to live more independently.
